The Nurse Navigator service at St Vincent’s NELUNE Men’s Health Centre is a care coordination service for people undertaking prostate cancer treatment at St Vincent’s Hospital, St Vincent’s Private Hospital, GenesisCare Darlinghurst and The Kinghorn Cancer Centre.
We know that the health system can feel complex and confusing at times – particularly during prostate cancer treatment. Our Nurse Navigator service helps you find your way to the quality health care you need in a connected, efficient, timely way.
Once you are referred by your doctor to the St Vincent’s NELUNE Men’s Health Centre our nurse navigator will become a contact at the centre. From your diagnosis right throughout your treatment, your nurse navigator will help organise your health care and support your decisions.
Through face-to-face meetings or virtual consultations, your nurse navigator will develop a care plan in collaboration with you and your health care team.
Your nurse navigator can coordinate care from our clinics, medical departments and other services.
